SECTION A  To be completed by Club requesting transfer. Then sent to player’s present club Secretary, using Recorded or Special Delivery.

 (Club must NOT approach player and obtain signature until Section B has been signed by the players current Club representative.)

 

Section B

 

To be completed by Secretary/Manager of Football Club with whom player is currently registered and returned to requesting Club within 7 days of receipt. Failure to respond within 7 days will be taken as acceptance of the request to make an approach.
